In-Network
I am in-network with:
> Traditional Medicare (the "Red, White, & Blue" card)
> BlueCross BlueShield of TN Medicare Advantage PPO, Blue Network P, and Blue Network S.
> Aetna (Commercial, Medicare)

Out-Of-Network
I may be able to accept some PPO plans. Check with your insurance provider before the evaluation to see if they offer out-of-network benefits and how to submit those claims. Such benefits may allow you to be reimbursed for some or all of your out-of-pocket expenses. 

A Word about Insurance....'cause it is confusing.
>Original medicare (the "Red, White, & Blue" card) does not require a physician’s order for a speech therapy evaluation, BUT your physician IS required to approve your treatment plan(known as a Plan of Care (POC)). It's best practice to talk with your physician about your desire for speech therapy BEFORE the evaluation.

>  Some commercial insurance plans (aka, insurance via your job) and medicare advantage plans require pre-approval --- or "prior authorization" --- before services can be provided. Please have your physician provide a signed physician order that I can submit to your insurance to complete this process. 

> You are responsible for all applicable deductibles, co-insurance and copays at time of service.

I do not have an office where therapy takes place. All therapy sessions are performed via telehealth, at the Client's home or a predetermined venue.

Not necessarily. If the client requires someone to help them in the bathroom with toileting needs, a caregiver competent to provide those needs is to be present at all sessions. In addition, for sessions completed out in the community, a caregiver is responsible for transportation to and from the activity.  RELIABLE emergency contact information needs to be provided if the client lives alone.

Currently, my hours are Monday /Wednesday /Friday. My earliest appointment is 9am, and the last appointment is at 5pm.
